Amlodipine

Amlodipine, a calcium-channel blockers, used alone or in conjunction with benazepril to treat hypertension (high blood pressure), chronic stable angina pectoris, and Prinzmetal’s variant angina.

Indications:
For treatment of high blood pressure, chronic stable angina, and vasospastic angina .

Dosage:
The dose administered orally (by mouth) of 2.5-5 mg, 1 time a day dose may be increased to 10 mg by mouth (orally), 1 time a day if needed. Maximum dose: 10 mg / day

Methyl prednisolon 16 mg

Methylprednisolone adalah suatu glukokortikoid sintetik dan diabsorpsi secara cepat melalui saluran pencernaan.

Methylprednisolone bekerja dengan menduduki reseptor spesifik dalam sitoplasma sel yang responsif. Ikatan steroid-reseptor ini lalu berikatan dengan DNA yang kemudian mempengaruhi sintesis berbagai protein. Beberapa efek penting yang timbul akibat ini yaitu berkurangnya produksi prostaglandin dan leukotrien, berkurangnya degranulasi mast cell, berkurangnya sintesis kolagen dan lain-lain.

Komposisi:
Methylprednisolone 16 mg, tiap tablet mengandung methylprednisolone 16 mg.

Indikasi:

Dosis:
Dosis awal methylprednisolone bervariasi dari 4 â€" 48 mg/hari (tergantung dari jenis dan beratnya penyakit, serta respons penderita). Bila telah diperoleh efek terapi yang memuaskan, dosis harus diturunkan sampai dosis efektif minimal untuk pemeliharaan.

Pada penderita yang baru mendapat methylprednisolone selama beberapa hari, penghentian obat dapat dilakukan secara mendadak. Untuk penderita yang mendapat methylprednisolone dosis besar selama lebih dari 1 â€"2 minggu, penghentian obat harus dilakukan dengan pengurangan dosis bertahap dengan tiap beberapa hari.

Dibandingkan dengan pemberian dalam dosis terbagi, methylprednisolone lebih baik diberikan dalam dosis tunggal pada pagi hari karena cara ini lebih sesuai dengan sekresi fisiologis hormon korteks adrenal.

Untuk penderita yang membutuhkan methylprednisolone dalam jangka waktu lama (lebih dari 2 minggu), dianjurkan menggunakan cara pemberian ADT (Alternate Day Therapy). Manfaat pemberian dengan cara ADT ialah mengurangi kemungkinan terjadinya efek samping berupa supresi sumbu hipotalamus-hipofisis-korteks adrenal, cushing syndrome, hambatan pertumbuhan pada anak dan gejala putus obat bila terapi dihentikan.

Rejimen dosis ADT ialah memberikan dosis total methylprednisolone untuk 2 hari sekaligus dalam 1 dosis (pada pagi hari). Pemberian obat dilakukan secara berselang seling yaitu hari pertama diberi obat, hari kedua bebas obat, hari ketiga diberi obat lagi, demikian seterusnya.

Cotrimoxazole 480 mg

Cotrimoxazole adalah bakterisid yang merupakan kombinasi sulfametoksazol dan trimetoprim dengan perbandingan 5 : 1. Kombinasi tersebut mempunyai aktivitas bakterisid yang besar karena menghambat pada dua tahap biosintesa asam nukleat dan protein yang sangat esensial untuk mikroorganisme. Tramadol 50 mg/ml

Tramadol adalah analgesik kuat yang bekerja pada reseptor opiat.
Tramadol mengikat secara stereospesifik pada reseptor di sistem syaraf pusat sehingga memblok sensasi rasa nyeri dan respon terhadap nyeri. Disamping itu tramadol menghambat pelepasan neurotransmitter dari syaraf aferen yang sensitif terhadap rangsang, akibatnya impuls nyeri terhambat.

Komposisi:
Tiap ml injeksi mengandung 50 mg tramadol hydrochloride.

Indikasi:
Efektif untuk pengobatan nyeri akut dan kronik yang berat, termasuk nyeri pasca pembedahan, nyeri akibat tindakan diagnostik.

Dosis:
Terapi oral
Dewasa dan anak di atas 14 tahun.
Dosis umum:
dosis tunggal 50 mg. Dosis tersebut biasanya cukup untuk meredakan nyeri, apabila masih terasa nyeri dapat ditambahkan 50 mg setelah selang waktu 30 â€" 60 menit.

Dosis maksimum:
400 mg sehari.

Dosis sangat tergantung pada intensitas rasa nyeri yang diderita.

Penderita gangguan hati dan ginjal dengan klirens kreatinin < 30 ml/menit:
50 â€" 100 mg setiap 12 jam, maksimum 200 mg sehari.

Terapi parenteral
Dosis yang diberikan sebaiknya disesuaikan dengan intensitas rasa nyeri. Bila tidak ada petunjuk lain dari dokter, dosis yang diberikan adalah sebagai berikut:

Dewasa atau anak di atas 14 tahun:
i.v.: 100 mg (1 ampul), diinjeksikan secara lambat atau dilarutkan dalam larutan infus, kemudian diinfuskan.

i.m.: 100 mg (1 ampul)

subkutan: 100 mg (1 ampul)

Dosis tersebut biasanya cukup untuk meredakan nyeri. Bila masih terasa nyeri, dapat ditambahkan 1 kapsul tramadol 50 mg atau 50 mg tramadol injeksi (1 ml) setelah selang waktu 30 â€" 60 menit. Pada penderita gangguan fungsi hati atau ginjal, perlu dilakukan penyesuaian dosis. Dosis maksimum 400 mg/sehari.

Indications:
symptomatic therapy rheumatoid arthritis, osteoarthritis, ankylosing spondylitis, acute musculoskeletal disorders and acute gout.

Dosage:
Adult rheumatoid arthritis, osteoarthritis and ankylosing spondylitis, an initial dose of 20 mg in a single dose. Acute gout, 40 mg daily in single dose for 4-6 days. Acute musculoskeletal disorders, 40 mg daily in single dose for 2 days, then 20 mg daily in single dose for 7-14 days. The dose for children is unknown.
